Appointments

Appointments are one on one with a physical therapist typically for 1 hour but may be longer dependent on the complexity of the condition. Scoliosis evaluations will typically be a two-hour appointment which is also our recommendation for ongoing appointments as appropriate for age and endurance.

  • Insurance card or claim information
  • Prescription from your referring provider if required
  • Disc containing x-rays for scoliosis patients
  • Reports of diagnostic tests that are pertinent to your treatment

Comfortable clothing that allows you to move freely. We prefer loose shorts if we will be performing soft tissue work on the lower body, otherwise flexible pants if we are focusing on movement. For shoulder, neck and upper back problems camisoles or sports bras for women are most appropriate.  Should you forget clothing we do have shorts and gowns available.

In the state of Washington, patients have direct access to physical therapy therefore a referral is not legally required. Most of the insurance networks in which we participate do not require a referral except for workers compensation claims.  We strongly recommend you contact your insurance company prior to your evaluation to confirm the specific requirements of your plan.

We strongly recommend, and may require based on the case, that x-rays be obtained prior to beginning treatment for scoliosis and hyperkyphosis. This provides the necessary information to develop a curve specific treatment program and rule out any contraindications for therapy. We need “Scoliosis View” x-rays. This is an x-ray taken in standing and includes the entire spine from skull to pelvis from both the front and side view.  Your primary care provider or specialist will need to order the test as physical therapists are legally unable.  For adults the x-rays should not be more than two years old, in children ideally not more than six months.  Please obtain a disc of your x-rays prior to your evaluation or you can request the disc be mailed directly to us as long as it will received prior to your first appointment.

We charge a $75 cancellation fee for appointments that are cancelled within 48 hours of the appointment unless there is an emergency or illness. If a two-hour appointment was reserved the fee will be $150.

We will verify the patient’s insurance benefits and can supply a written good faith estimate for cost of treatment. It is ultimately the responsibility of the patient or the guarantor to confirm and understand their policy requirements and benefits. This includes deductibles, copays, annual visit maximums, daily treatment time limits, and pre-authorization or referring provider requirements. The patient or their guarantor is responsible for payment of claims the insurance does not process within 90 days of submission.

Many Regence Blue Shield and Premera Blue Cross of Washington plans require authorization for physical therapy treatment beyond a specified number of visits. Authorization requests are submitted by the physical therapist not the referring provider. The insurance carrier will approve additional therapy based on medical necessity regardless of available benefits or referring provider orders.

Some insurance carriers including Premera Blue Cross of Washington and Aetna have a daily maximum treatment time of one hour. Any treatment provided beyond this limit will be the responsibility of the patient as a cash payment
billed in 15 minute increments.


Payment

Payment for copays and cash pay clients is due at the time of service. We accept credit card payments and checks.

Our cash rate is a 25% discount of our fees submitted to insurance: $200 for a 1-hour evaluation and $150/hour for treatment. Our appointments are a one hour minimum, and extended appointments of 1.5 or 2 hours are available. Duration of physical therapy is based on each individual patient and their response to treatment. The therapist can give an estimate  regarding the length of treatment following the evaluation.
